Fund Designee & Donor Advisor Support

This page is specifically for fund advisors.  You may access forms that you need in order  to support your particular fund’s work below.  In the future we plan to add helpful tips and directions about making grants, etc. If we have forms that  you may need in order to support that can’t be downloaded from the list below or if you have any other questions you may contact our staff at any time.  

 

Form: 

· Grant/Check Request Form— for use by a Donor Advisor of Fund Designee when you want to request that a check be cut from a fund.  (***New Version: 10-21-08****)

 

· Request to Establish a Fund— to be completed by organization or group who would like to request and establish a fund within the Foundation.

 

· Request to Add/Change Fund Advisors download this form if you would like to add or remove a fund advisor to the fund.  Should be completed by current fund advisors).
